When I was in kindergarten, I distinctly remember my first school assembly. When the 8th graders walked in. I was star struck. To me, they were as big and as OLD as any grown up I knew. People in their 40s and older, well I wouldn’t have been surprised to see them as guest stars on “The Golden Girls”

Fast forward a “few” (ahem) years to today. I most certainly do NOT feel or look ready to star in any reboot of “The Golden Girls”. Generations of the past seemed to embrace “seniority” far sooner than we do today. Predictably, more and more people are turning to help in effort to accomplish this. As a board-certified plastic surgeon, this is “MY LANE”. I am here to tell you, THERE. IS. HELP.

If you do nothing else for yourself, invest in your skin. In Florida you are constantly being exposed to the sun. NOTHING ages your skin more than the effects of the sun’s ultraviolet rays. The majority of damage was done years ago, back when you thought you were invincible, and also did all that other stuff that we don’t need to discuss...

Apply SUNSCREEN every morning. EVERY morning. Many makeups now have UV protection in them. SPF 15 at least, more if you plan on being outside for any extended period.

For damage already done, Physician grade skin care is a must. The effects of sun damage, smoking, dehydration, soap, etc., can make your facial skin appear older than it actually is. Outrageous! The skin care treatments you can buy online or over the counter may have worked in the past but are no longer doing what they once did. Prescription strength (that you can only purchase at a physician’s office, or medical spa with a physician as a medical director) skin care products will actually restore your skin, on a cellular level, back to that of a more youthful state. Your skin doesn’t just look healthier, it IS healthier! Granted, these changes don’t happen overnight. A good skincare will take at least 8 weeks (realistically 12-16 weeks) to see actual changes. This is because it’s only affecting the newly growing cells. Coincidentally, it takes approximately 8-12 weeks for the skin cells you have to be completely replaced by new cells.

If you don’t have 8-12 weeks to spare, consider a jump start with a resurfacing procedure. The most common ones are either a chemical peel (glycolic, salicylic, or TCA), or laser resurfacing. Many of these can be performed in an office with minimal (0-2 days) downtime. There are more aggressive peels/lasers/plasma resurfacing procedures, which can show results in just one procedure, but you will have to plan for more downtime from work/exercise/sports. Keep in mind, the quicker the downtime, the less aggressive the procedure will be and the less dramatic the results. This isn’t necessarily a bad thing, just be prepared to have more than one.

Breast augmentation has been one of the most popular elective procedures performed for women for decades. Many times however, when a woman makes a consultation for a breast augmentation, she may in fact need a breast lift, or an augmentation and a lift. Understanding the difference between breast augmentation and breast lift can sometimes be confusing. Hopefully this will help.

A Breast Augmentation (mammaplasty ) enlarges the size of a woman’s breast.
A Breast Lift (mastopexy) reduces the dependency (sagginess) of a woman’s breast, but the breast size stays approximately the same.
An Augmentation/Mastopexy is both procedures performed together.

There are different ways to perform a breast lift, depending on how much lift needs to be done. If a woman who wants larger breasts also needs a lift, the amount of lift required can be influenced (to some degree) by the size of the implant she chooses. A larger implant may result in less lift or even no lift required. This is not to say that a large enough implant can forego the need for a lift entirely! This can sometimes be achieved, but if done incorrectly or carelessly it can also result in a size much larger than the patient wanted or anticipated! Many plastic surgery offices offer modalities (physical sizing systems, 3D imaging, etc.) that can predict the result of a breast augmentation. Each method of sizing has specific benefits. Preoperative sizing is very important in order to make sure the patient understands and agrees with what size she will be after surgery. A patient should be involved in the decision making process regarding implant size, shape, style, and location. A thorough consultation with examination and sizing will help ensure that a woman will have an informed understanding of the anticipated result after surgery, and reduces her risk of requiring revision for an incorrect size (too large OR too small).

In summary:

If a woman likes her breast SHAPE but wishes her cup size was LARGER, a breast augmentation would be best. This procedure is most common in younger patients with minimal dependency (sagginess) to their breasts.
If a woman likes her breast SIZE but wishes her breasts were PERKIER, a breast lift would be best. This procedure is often chosen by women who have had children and have had changes in their breasts from pregnancy/nursing.
If a woman wants both LARGER and PERKIER breasts, an augmentation/mastopexy would be best. This procedure is also often chosen by women who have had children for the same reason.

Cosmetic Fillers and COVID-19 Vaccine

Well, 2020 ended at the stroke of midnight on December 31, but for whatever reason, life as we know it hasn’t changed much. Weren’t we all hoping life to magically return to normal as the ball dropped in Times Square?

Fortunately, there is hope at the forefront. Due to the fantastic work of great scientific minds, we now have available novel vaccines that may finally allow us to get control over this pandemic. In the United States, there are currently two vaccines available from the companies Pfizer and Moderna. Both are mRNA vaccines, which are an exciting new approach to fighting a disease that I will allow a genetic scientist, virologist, infectious disease specialist, but not a plastic surgeon like me, to explain. The Moderna vaccine will likely be the most widely available due to its less stringent refrigeration requirements than Pfizer.

However, recently, infrequent reports that some individuals with injectable dermal fillers may experience swelling in the areas where the fillers were injected upon receiving the vaccine. Leading experts in the field of injectable fillers were consulted and noted the following:

1) Reactogenicity (expected adverse reactions from a vaccine) occurs after vaccines are given and are indicative of the body’s early response to that vaccine up-regulating the immune system and usually lasting about two to three days. Reactogenicity from any vaccine commonly ranges from mild injection site arm pain, redness or swelling, but can also cause more general symptoms like fever, lethargy, malaise, and muscle aches. In the cases of dermal fillers, it seems that the facial swelling resolved within a couple of days. Generally, this can be treated with antihistamines or oral steroids.

2) Similar incidents of this type of facial swelling have been previously reported in filler patients after other vaccines (like flu vaccine or shingles vaccine)

3) If someone who has had previous dermal fillers has the opportunity to get the COVID vaccine, they should take it as the potential benefit of the vaccine is vast, while the risk of a mild, short-lived, and treatable facial swelling is very low.

In short, the risks associated with COVID vaccines and injectable fillers are not significant enough to prevent individuals who have the opportunity to get vaccinated from doing so.

Suppose an individual was to experience facial swelling after COVID vaccination. In that case, they should contact their board-certified facial plastic, oculoplastic, dermatologist, or plastic surgeon accomplished in dermal fillers and up to date in the treatment of adverse reactions.

Implants have been used in the past (and are still used sometimes), much like they are in breast augmentation procedures but have decreased in popularity with newer techniques.

Brazilian Butt Lift is the term typically used to describe making one’s bottom look more extensive and more contoured. This involves removing fat from other areas via Liposuction, washing and preparing the fat in a sterile fashion, then reintroducing the fat into the butt to increase its volume. Liposuction and fat transfers in small volumes can safely be done in an office setting. Still, larger volumes of Liposuction and fat transfer should be done in an accredited surgical facility (either an office-based surgical suite or an ambulatory surgery center). Fat transfer is not a perfect science, however, and not all the fat will survive. Most patients can expect to lose anywhere from 25-50% of the immediate postoperative volume as they heal due to inadequate blood supply to the transferred fat.

As Brazilian Butt Lifts have increased in frequency and popularity; however, it was noted that there was an alarming number of deaths resulting from improper techniques! The unusually high mortality rate was estimated to be as high as 1:3000, more significant than any other cosmetic surgery. Due to the risk of performing the Brazillian Butt Lift, Dr. Wallace has chosen not to offer this procedure to his patients. However, we have found alternative methods such as body contouring.

Dr. Wallace uses body contouring to diminish the prominence of areas around the buttocks (lower back, hips, upper thighs) using Vaser liposuction. More like sculpting with marble, it is what you leave behind that gives you the aesthetic result. Many women already have a shapely backside, but it is masked by excess fat above, below, and sometimes on the sides of the buttocks. Reducing fat in those areas gives the buttocks a more dramatic appearance and shape, without the risks associated with fat injection.

Thigh Lift Recovery

Introduction

I had excess skin removal surgery completed by Dr. Wallace as a result of losing nearly 200 lbs. I had skin removed from my chest, abdomen, and inner thighs. My chest and abdomen recovery were both pretty uneventful so this will primarily focus on the inner thigh recovery process I experienced, since it was a bit more involved. Dr. Wallace forewarned me that the thighs would be the most difficult recovery process due to the general high functioning area where the incisions are located and due to the area also being generally humid and damp. I tried to find some resources on this recovery process online both before the surgery and during my recovery process but could not find anything to really prepare me for, or help me through my experience, so I wanted to make this to help others understand what theirs might be like. Some things I learned:

As Dr. Wallace will tell you, and told me...Blood is good, it is what brings healing to the areas, so don’t be too concerned when there is a lot of blood or more blood than you might have expected during the recovery process.
The healing process is not linear. There will be times of rapid improvements, times of seemingly no improvement, and periods of what seems to be steps backwards. Trust Dr. Wallace, the process, and your body, knowing that you will recover and see great results in the end.
Compression garments and wound dressings can not be too tight. This can cause swelling in weird places and prevent blood flow to your wounds that can slow/hinder the healing process. Pressure similar to a hug is the optimal amount for blood flow and healing.
Stay hydrated, eat plenty of protein and calories via as much whole foods as possible, and get as much rest as possible.
Don’t overdo the activity too early in the recovery process. Ease back into it.
Be careful when attempting to sit on the toilet, buy a commode and use that since those are higher, it will be way less painful. Especially if you have old toilets that are really low.

At this point I have had 5 of 6 drains removed and am able to move around enough that I am showering myself and able to bend over enough to sort of inspect my thigh incisions on my own. I had noticed quite a bit of blood on my toilet seat after my bowel movements and wanted to make sure everything was ok with my incisions during my shower, so I took some pics. I was a little concerned at this point, because the incisions were not fully closed and to someone not in the medical field or familiar with wound care, they looked rough, especially compared to how tightly closed all my other incisions were. Dr. Wallace responded promptly to my text on Sunday night (even though he was on a camping retreat with his son) and assured me the incisions look normal and the separation was nothing to worry about.

6/9 (13 Days Post Op)

I went to shower today and when I was applying my antibiotic ointment to my thigh incisions, they felt much bigger than I remembered and decided to inspect them. As you can see in the pictures the open wound is much bigger than just 3 days prior (ignore the white cream that is the antibiotic ointment I applied). Again, being unfamiliar with this process I was a bit freaked out. I sent pics via text to Dr. Wallace, he again reassured me this was normal, but wanted to see me anyway to put something on the wounds to help the healing process. I was able to see him the next day where he removed my final drain and applied something to the wounds. It stung a little, but not bad at all.

6/12 (16 Days Post Op)

7/6 (40 Days Post Op)

At this point I was cleared to start working out my upper body at the gym since all my other incisions were in great shape. The thighs were slowly getting better, but I started to notice that my left thigh incision was opening back up where it previously seemed completely healed and fully closed. This was concerning because it seemed like an obvious step backwards in the healing process. Also, my right thigh, which had improved and was healing, seemed to stall and was still a pretty large wound. I messaged Dr. Wallace about this on 7/7, he assured me, again, this was normal but wanted me to see him on 7/9. What was really interesting was that from 7/6 to 7/9, somewhere in there the thigh incisions stopped bleeding, especially the right one and seemed to rapidly start healing. Then on 7/9 Dr. Wallace applied that same treatment to my wounds to promote healing and had me come back in 2 weeks on 7/27.

7/26 (60 Days Post Op)

I had an appointment scheduled for 7/27, but with my thigh incisions healing so well I messaged Dr. Wallace to see if pictures would suffice. Once Dr. Wallace saw the pictures and he was pleased with the progress and agreed that the appointment was unnecessary. At this point I stopped putting any sort of bandage or dressings on these wounds and they healed up perfectly. I am not sure at what exact date they were completely closed because from here until that point, I did not feel or notice any differences or any changes and was not inspecting them regularly. I would pay more attention to them when showering but can’t recall noticing that they were fully healed on a certain date.

Here are some pictures of what they look like today, as I write this. I could have probably taken fully recovered “after” photos with the same result about a month ago on 8/5 which would have been exactly 10-week post op, but I was busy getting ready for my wedding and then was on a 2-week honeymoon, so this was not top of mind at the time.

Q: When will you be able to wear underwire bras?

A: You will have to wait a month or two to start wearing underwire bras depending on what Dr. Wallace says. In the meantime, wear sportswear bras or the bra that Fleming Island Plastic Surgery offers. We know how much you’re looking forward to wearing all your cute new bras, but this rule is in place so that you have the best results! For your wounds to heal properly, it is best to wait until Dr. Wallace gives the O.K. on wearing underwire bras.

Q: When can you start exercising again?

A: Start off by taking slow walks the day after your surgery, then you can start gradually exercising more but only as instructed by Dr. Wallace. Do Not overdo it!

Q: Will you be able to breast feed after this procedure?

A: Yes, you usually can breast feed with breast implants. Talk to your surgeon or OB/GYN if you have any concerns.

Q: Will I still be able to have mammograms?

A: Yes, let the technician taking your mammogram know that you have breast implants. It is important to let them know so they can use special methods on taking the mammogram to get the quality x-rays needed for the radiologist.

Q: Can you still detect breast cancer with implants?

A: With the sub-muscular placement of the implant, or under the pectoral muscle, you can still view about 93% of the breast tissue. With the sub-mammary placement, or above the pectoral muscle, there is an 87% view of the breast tissue with a mammogram

CHOOSING THE RIGHT SIZE IMPLANT

When choosing what size of implant, you want, our #1 recommendation is, don’t think about bra/cup size. There are several reasons why we say this.

Cup sizes tend to change for all women from one bra manufacturer to another;
Many women have never been professionally fitted for a bra and therefore wear the wrong size already;
Implant size does not really correlate to cup size;
Many women will choose a smaller implant solely to avoid cup size stereotypes;
The shape of the implant impacts your desired look as much as the size.

Suggestion: Take time to play with implants that you think are “way too big for you.” We have sizes from 125cc to 500cc. (cc refers to the volume of fluid filling the implant). Why play with large implants you ask? Reports show that 90% of women regret not choosing a larger implant. Get yours right the first time! No reason to pay for two surgeries or to put your body through a second procedure because of a silly stereotype.

Dr. Wallace has two goals when he has you try on sizers.

Find a size you LOVE and then
Find a size that’s too big so later if you’re second-guessing your size choice, he can remind you that you didn’t like the next bigger size.

CHOOSING THE RIGHT SHAPE & TYPE OF IMPLANT

Don’t worry. Dr. Wallace and the FIPS team will help you choose what’s right for you. You will have as much time as you need, even multiple visits if necessary, until you feel sure you have picked the perfect implant.

Types of implants: Silicone vs. Saline

Saline implants consist of a silicone outer layer with a sterilized salt water solution that is filled after placed in the breast.

Silicone implants are prefilled & have a thicker gel-like consistency. They are more natural feeling as compared to saline implants.

Both types are safe, and both offer certain advantages. During your consultation, Dr. Wallace will go into more detail and answer any questions you have.

Shapes of Implants: Teardrop vs. Round

The implants can come in a teardrop shape, which makes the breast appear more naturally shaped, or the choice of a rounded shaped implant, which typically makes the top of the breast appear fuller.

Understanding Board Certification

Make sure the surgeon you choose is board-certified in PLASTIC SURGERY by the American Board of Plastic Surgery. The American Board of Plastic Surgery website will tell you if a doctor is board-certified in plastic surgery. www.abplasticsurgery.org

Why is it so important to have a board-certified surgeon? Because legally any surgeon is permitted to do any procedure. That means even an oral surgeon could legally perform a breast augmentation. And yes, this kind of thing happens in the real world! Don’t be fooled into thinking that all board certifications are equal because they aren’t.

Requirements to become Board Certified in Plastic Surgery are very strict and include the following:

Graduate from an accredited medical school;
Complete at least six years of surgical training following medical school with a minimum of three years of these in a plastic surgery residency training;
Pass comprehensive oral and written exams;
Perform surgery in accredited, state-licensed, or Medicare-certified surgical facilities;
Follow a strict Code of Ethics;
Continuing medical education, including patient safety.

Check the surgeon’s record & ask about hospital privileges

Be sure to check the State of Florida Medical Board’s website which can be found at www.flboardofmedicine.gov/.

Look for malpractice judgments, plus any disciplinary actions by the board.
It never hurts to do a Google search too!
Even if you’re having a plastic surgery procedure at an outpatient clinic that is not affiliated with a hospital, it’s worth asking where the doctor has hospital privileges, because hospitals do background checks. If a surgeon doesn’t have hospital privileges somewhere, that’s a red flag.

The Squeamish Need Not Apply

For sure, if you decide to have a cosmetic surgery, you will need a caregiver immediately following your surgery and for a few weeks after. Since that person (or persons) will play a very important role in your recovery, we want to give them a few good tips. Be sure to share this list with anyone that you think may be helping you after surgery!

TIPS FOR CAREGIVERS

First, if you’re squeamish about blood, you may not be a good person for this job. Most of the time, drains are involved in this procedure. Drains help to remove the buildup of blood and fluid during the healing process. These drains may need to be emptied a few times and usually, the output needs to be measured and reported to the surgeon.
Do not be a Debbie Downer! Be positive and supportive. This is not the time to make her question the decision to have surgery. Please be supportive and remind her that the recovery process is temporary, and it will be worth it in the end!
Know the warning signs so you know when to call for help
Do not let her overdo it. Many women have a hard time asking others for help. Know the limitations given to her by the surgeon and make sure to assist with bathing, dressing, emptying the drains, household chores, meal preparation, kids, and pets. Help even if she doesn’t ask.
Encourage her to walk around frequently. Short walks, even the same day as surgery, help to avoid complications like DVT.
always Remind her to keep the compression garments on or as instructed (see post-op instructions).
Assist with getting up and down and provide support while showering.

LET'S TALK ABOUT SEX, BABY!

Depending upon which procedures you have done, your post-surgery sex limitations will vary. You should always ask Dr. Wallace about your specific limitations and be sure to follow them exactly. The following will give you a good idea of what you can expect so you can prepare your partner for the inevitable sexual dry spell.

With a tummy tuck, you can expect not to have sex for at least 2 weeks after surgery. Once you’re released to return to sexual activity, go slow and let your body be your guide. Please, don’t overdo it!

If you are having a labiaplasty, it is usually recommended that you abstain from sexual intercourse for 4-6 weeks.

Even after Dr. Wallace tells you it is ok to have intercourse again, you will want to use a high-quality lubricant and only “make love.” No rough sex. Your partner will need to understand that you are in charge, and of course, let your body be the guide.

Do not do anything that causes discomfort or pain. Just be patient. Eventually, things will get back to normal.

Many people assume that anyone that gets fillers will look overdone and crazy. Believe me, that is what many people that are not familiar with fillers associate it with. They see Kylie Jenner’s lips or women on the ever so popular Housewives shows on Bravo and its true, you can tell they have had their lips done. What you don’t notice are the men and women that you may encounter every day. Many people do not share their experience with others for fear of judgement. Those individuals may get fillers to add subtle volume to lessen the lines of their face, or maybe just to add volume and/or definition to their lips. If done correctly, you shouldn’t be able to positively identify that filler has been injected.

Well, from my experience lips have been popular among all ages to add volume. Some individuals with thin lips desire a fuller lip and others with moderate fullness may desire more volume or just definition. Everyone’s lips are not the same shape or size and during our assessment, we will give you a clear explanation of our treatment plan based on your needs. If you have a pair of lips that you desire to emulate, we can discuss if that result can be attainable based on realistic expectations. Also, if you have thinner lips and desire a much more volumized lips, we will start off with one syringe and have you come back after the swelling subsides before adding anymore. Many times, individuals are happy with one syringe.

So, I feel like I have lost volume in my cheeks. Is there a non-surgical solution for that?

Yes! Fillers come in many different forms and are meant for different areas on the face.

Dermal fillers are products that are placed in the face for added volume. It is typically placed in the lower 2/3 of your face for example the cheeks, nasolabial folds, lips, and marionette lines.

As we age, our cheeks lose subcutaneous fat and bone which can cause our skin to sag, wrinkles, and give us the unwanted droopy jawline (jowls). Dermal fillers improve lines immediately after injection, however, swelling occurs for 48-72 hours. You may not see the final results for at least one week after the swelling and bruising subsides.

There are so many options, how do I know which one is right for me?

The appropriate filler choice depends on you desired aesthetic goals. Your provider will then be able to choose the right product for you. For example, we have two options for cheeks. Radiesse and Voluma. Radiesse is placed on the cheek bone because its calcium hydroxyapatite ingredient in the gel’s consistency is like human bone. You wouldn’t want anything soft, palpable or movable in your cheeks. This consistency offers smooth and firm results that lift and add volume. Voluma is another product we offer for the cheeks and is also injected similarly.

How long do fillers last?

For cheeks the products can last anywhere from 18-24 months. For the lower face, those fillers can last anywhere from 6 -12 months due to location. The areas around the mouth or lips are always moving with talking and eating, which metabolizes more quickly than the cheeks or mid face. Every person is different, and results may vary.

When is the right age for a person to start Botox or Xeomin injections?

As injectors, we are asked that so many times that I have started to lose count. There is not one definitive answer. It depends on the individual because there is no magic number dictating what age is appropriate and every individual is different. What I can tell you is that treating the wrinkles before you get them is more effective. So, why is it more ideal to start early as a preventative? Well for starters you will require less product to treat if you don’t already have the lines and secondly, it prevents the lines from forming. Your wallet and selfies will thank you later…I promise!

Another great answer for the right age is when those lines start to bother you and I have individuals that start in their twenties and some that start many years later, and that’s ok! Only the individual can choose when that time is right for them.

So, what is Botox and Xeomin?

I have been asked by family, friends, patients and new people that I meet “What is Botox?” and “Can I put it in my cheeks or buttocks?” I cannot make that last one up, haha!

First of all, Botox and Xeomin are a class of medication called neuromodulators, which does not add volume like fillers. We will get into what fillers are a little bit later. Neuromodulators, like Botox and Xeomin are placed in the crows feet area at the corner of your eyes all the way to the forehead. So, there you have it ladies and gents Botox is not for the buttocks or cheeks!

Neuromodulator is a big word, so what does it mean!?

At Fleming Island Plastic Surgery, we offer only Botox and Xeomin (And yes there are many brands out there, but these are our two favorite!) Botox and Xeomin are two different brands by two different companies that have similar active ingredients. These neuromodulators are used to temporarily improve the appearance of lines that occur at rest and prevent new lines by blocking the neuromuscular activity of the muscles. Basically, it relaxes your muscles that the medication is injected into so that those lines do not occur. With less muscle activity, the lines improve with disuse, however, the product’s effect only lasts about 3 months. It is not permanent; therefore, you do have to be retreated every 3 months for best results.

Alright, where is this magical substance injected?

Typically, we inject neuromodulators to treat areas with repetitive motion such as raising the eyebrows, frowning (with the eyebrows) and squinting. The repetitive motions result in horizontal lines of the forehead, elevens (the vertical lines in between our brows) and lines around the eyes. There a few other places that it can be injected but those are considered “off-label”.

Are the results immediate?

No, it takes about 5-7 days for the injections to take full effect. The injections themselves are quick and painless because we use a very small needle. We inject small doses at multiple injection sites. You will notice decrease in movement in about a week so you will not see the effects right away. The effect will last 3 months, but with more regular treatment the better your results will be. It is a commitment to maintain wrinkle free.

Common misconceptions #FakeNews

Another misconception I have heard is that once you start Botox you have to keep getting it because you will look worse when you stop. This is #FakeNews!! Neuromodulators cause your muscles to relax which improves those pesky lines, but when you are used to getting regular treatments for years and then stop. The lines can come back if not treated and you may notice it more. So, if for some reason you get off track with your treatments, don’t stress we can get you right back to wrinkle free when life slows back down!

Botox seems to be used in plenty of movies and in a joking manner. Also, the media like our favorite celebrities seem to overdo things which can cause you to have the wrong idea.
